RESUMO

CONTEXT: A central recommendation from the Pew Health Commission to educators has been to empower future care providers to function effectively as teams. Administrators and faculty members within a school of allied health sciences thus established an interdisciplinary program where students would learn to function as team members and demonstrate competencies required for practice in diverse, demanding, and continually changing health care environments. Students from five disciplines have participated in featured events, mentored activities and capstone projects, earning credit in an interdisciplinary course of study that complements offerings in their home disciplines. GOAL: This follow-up article reports on the progress and development since 2002 of an interdisciplinary program known as Team IDEAL. Formative evaluation measures used to assess satisfaction with the program are presented alongside a discussion of new directions. CONCLUSION: Team IDEAL will move forward in a streamlined form that reflects its central aim. IDEAL leadership will remain cognizant of the effects of discipline-specific curricular changes, complex programming, and student perspectives on the process interdisciplinary education.

PURPOSE: Previous works show a pronounced disagreement on the reliability of stereo tests as screening tools for amblyopia and strabismus. This study's aim was to compare the ability of the Lang II, Frisby, Randot, Titmus, and TNO stereo tests to detect amblyopia and strabismus with visual acuity testing and cover testing. METHODS: A total of 1035 school children aged 12 to 13 years were examined in a field study in Monterrey, Mexico. In addition to the 5 stereo tests, the examination included visual acuity, cover testing, refraction (skiascopy), and inspection of the red reflex and posterior pole. RESULTS: Sensitivity ranged from 17% to 47% (Frisby-Titmus-Lang II-Randot-TNO, in order of occurrence). Of the 60 subjects with strabismus and/or amblyopia, only 8 subjects were identified by all 5 stereo tests. A considerable number of subjects (25), the majority of whom were amblyopic (23 subjects), were not identified by any of the tests. All stereo tests showed higher sensitivities for strabismus than for amblyopia. CONCLUSION: None of the 5 stereo tests studied is suitable for screening for amblyopia or strabismus. The results of both ocularly normal subjects and subjects with strabismus and/or amblyopia are variable, and there is no way of separating normal response from abnormal response.

RESUMO

Hypermetropia of more than 5 diopters or an astigmatism of 2 diopters or more is often the cause of amblyopia in the master eye, independent of the presence of a simultaneous strabismus or anisometropia. This amblyopia can be prevented with early spectacle correction. In the authors' study of 218 patients with high ametropia a correlation was not always present between visual acuity and the age of the first spectacle correction or the degree of ametropia. In some patients even correction of ametropia as late as seven to ten years of age led to spontaneous functional improvement of single optotypes acuity. The treatment of choice is to give the full ametropic correction at an early age.

The visual acuity values obtained with the four test figures of Lithander's "Kolt-test" have been compared with Landolt ring acuity under standardized conditions which meet the requirements of DIN 58,220 and ISO/DIS 8596/7. For a given test figure size, the Kolt-test optotypes turned out to be somewhat easier legible than the Landolt ring. On average, visual acuity measured with the Kolt-test exceeded Landolt ring acuity by 0.08 log (angle of resolution) units. Thus, according to DIN and ISO criteria, the legibility of the Kolt-test is not equivalent to the standard Landolt ring. Equal acuity values could be obtained by increasing the observation distance proposed by the manufacturer of the Kolt-test by 19%. The legibility of the four optotypes differs on adult subjects: "circle" is more difficult, "cross" is easier legible than "square" and "triangle". Confusion of Kolt-test figures is basically restricted to two pairs of two optotypes ("cross" - "triangle", "square" - "circle").
